The Montevideo clinic is open five days a week to provide a variety of services including <a href=\"https:\/\/ccmhealthmn.com\/service\/general-primary-care\/\">general care<\/a>, <a href=\"https:\/\/ccmhealthmn.com\/service\/general-primary-care\/\">family practice<\/a>, <a href=\"https:\/\/ccmhealthmn.com\/service\/obstetrics-and-gynecology\/\">prenatal and obstetric care<\/a>. Our hours of operation are 8:00am\u20146:00pm Monday through Friday.<\/p>\r\n\r\n\r\n\r\n<p>Our team of health care <a href=\"https:\/\/ccmhealthmn.com\/find-a-doctor\/\">providers<\/a>, nurses, and other medical staff care about providing compassionate and personalized health services. We consider your health needs, medical history, concerns, and health goals to ensure you receive excellent care. Our facility is equipped with modern medical equipment, and our staff has up-to-date training. Above all, we&#8217;re committed to high-quality health care that puts your needs first.<\/p>\r\n\r\n\r\n\r\n<p>Our Montevideo Clinic provides physical, emotional, mental, and social health services, including:<\/p>\r\n\r\n\r\n\r\n<ul>\r\n<li>Routine appointments<\/li>\r\n<li>Physical and annual exams for all ages<\/li>\r\n<li>Sports physicals<\/li>\r\n<li>Prenatal and obstetric care<\/li>\r\n<li>Child wellness visits<\/li>\r\n<li>Blood pressure checks<\/li>\r\n<li>Immunizations<\/li>\r\n<li>DOT exams<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p>Once you schedule an appointment, it&#8217;s important to arrive 20 minutes early so you have time to register and speak with a nurse. Please bring the following items to your appointment:<\/p>\r\n\r\n\r\n\r\n<ul>\r\n<li>All current medical insurance cards<\/li>\r\n<li>Your Driver&#8217;s license or photo ID<\/li>\r\n<li>Your Social Security card<\/li>\r\n<li>All medications, herbal treatments, and supplements in their original bottles<\/li>\r\n<li>A list of all known allergies<\/li>\r\n<li>Any forms, documents, or papers you need a medical professional to complete<\/li>\r\n<li>A list of questions or concerns so your health care provider can help address them<\/li>\r\n<li>Unaccompanied minors must also bring a signed authorization form to receive treatment.<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p>If you need medical services due to a workman compensation injury or an automobile accident, our staff is here to help.
