Please bring the following items to your appointment:<\/p>\r\n\r\n\r\n\r\n<ul>\r\n<li>All current medical insurance cards<\/li>\r\n<li>Your Driver&#8217;s license or photo ID<\/li>\r\n<li>Your Social Security card<\/li>\r\n<li>All medications, herbal treatments, and supplements in their original bottles<\/li>\r\n<li>A list of all known allergies<\/li>\r\n<li>Any forms, documents, or papers you need a medical professional to complete<\/li>\r\n<li>A list of questions or concerns so your health care provider can help address them<\/li>\r\n<li>Unaccompanied minors must also bring a signed authorization form to receive treatment.<\/li>\r\n<\/ul>\r\n\r\n\r\n\r\n<p>If you need medical services due to a workman compensation injury or an automobile accident, our staff is here to help.
